You will be required to mail or fax copies of these items for each student:

• Proof of Residence (such as a city utility bill)

• Official Birth Certificate

• Current Immunizations (or signed exemption)

• Special Education Papers IF APPLICABLE (IEP, 504)

This pre-placement session is presented online for your convenience.  It will cover information needed for preparing for the school year and selecting the courses. 

Complete this form to indicate that you have completed all steps up to this point and are ready to select your courses.

You will be contacted by a WOSU teacher who will schedule a placement conference with you and walk you through the rest of the process.  This conference/call will take place online and/or by phone.  Together with the teacher, you will select the desired courses according to your child’s learning level and abilities

Yea!!  Your portion of the enrollment process at this point will be complete and everything else will be taken care of by the WOSU staff.  Your child's courses will be ordered.  The online school will be activated and the materials will start to arrive. 

You are welcome to begin working on the classes online after August 1st.  The offline portions can be done at any time after you have received the materials.  School will officially start on August 1th.  At that time a WOSU teacher will be assigned to you to help and support you in your educational endeavors throughout the school year.
